An appropriate qualification that allows for the required registration with the South African Council for Social Service Professions (SACSSP) as a Social Worker• Registration with the SACSSP• Applicant must be in a possession of a valid South African driver's license• A fully completed Z83 form, and a comprehensive CV should accompany the application. Other skills/requirements: Knowledge of applicable and relevant legislations and policies in the management of health conditions, legal and ethical practices related to the field of Social Work, Public Finance Management Act and Public Service Act and Regulations• Good verbal and written communication, interpersonal, computer literate, financial and human resource management skills. Self-organization: planning, innovation, time management and ability to work independently and in a multidisciplinary team context • Analytic thinking, independent decision making and problem-solving skills. Interest and experience in community health services will be an added advantage.
Duties :

Provision of integrated therapeutic psycho-social services to the service users of all age groups, their families and the community using all methods of social work including prevention, early detection, screening and the promotion of wellbeing and quality of life.
Establish intersectoral collaboration liaising with other provincial departments such as Depts. of Home Affairs, Social Development, SASSA, SAPS, Non-Governmental Organizations, Traditional healers and Faith-Based Organizations in the district to promote seamless delivery of efficient and effective psycho-social services. Collaboration and communication with other PHC staff, the WBOTS program, and other district health services (HAST, Clinical Forensic Medical Services, Mental Health, Child Care Services) to integrate psycho-social services into their service delivery objectives. Establish referral pathways at all levels of care and liaise with district regional and tertiary hospitals in the clusters. Provision of training and support to general health staff and community health workers. Attend and participate in clinical meetings and supervision sessions. Administrative duties, which include clinical process notes, daily and weekly schedule / diary management, compilation and submission of statistics and monthly report (daily and monthly.
